RESUMO

Los sistemas personalizados de dosificación (SPD) constituyen un recurso primordial para mejorar la adherencia farmacoterapéutica desde la farmacia comunitaria.El farmacéutico comunitario (FC), al ofrecer el SPD como servicio profesional farmacéutico asistencial, debe aprovechar todo su potencial para alcanzar objetivos terapéuticos, además del acondicionamiento de la medicación en el blíster desechable, e incorporar Revisiones del Uso de los Medicamentos (RUM), seguimiento farmacoterapéutico y un contacto frecuente con familia y demás equipo de salud que atienden al paciente. En este caso, conscientes de la importancia de un seguimiento cercano, especialmente durante el confinamiento por la pandemia de COVID-19 y en una paciente con enfermedad de Alzheimer, se consigue mejorar la adherencia terapéutica gracias a un servicio farmacéutico que, más allá del blíster, incluye entrevistas con cuidadora y familia, revisiones del uso de medicación semanales, indicación farmacéutica y contacto directo con el médico de atención primaria (MAP) en situaciones complejas como, por ejemplo: aparición de nuevos problemas de salud y reacciones adversas a los medicamentos (RAM), provocados por medicamentos o dificultad para tragar sólidos. En este caso, debido al aislamiento provocado por el confinamiento, se incluyen además servicios de Atención Farmacéutica Domiciliaria (AFD) y dispensación a domicilio. Protocolizar y realizar una revisión de mediación semanal en el servicio de SPD mejora la adherencia y la consecución de objetivos terapéuticos. La colaboración entre médico, farmacéutico y familia facilita el seguimiento de los pacientes adheridos al servicio para conseguir resultados en salud. (AU)

INTRODUCTION: Nowadays the study of vulnerability to psychosis and early intervention is an area of great clinical impact and research. The aim of this study was to adapt the Wisconsin Scales of Vulnerability to Psychosis from English to Spanish. A set of five scales (Magical Ideation, Perceptual Aberration, Physical Anhedonia, Revised Social Anhedonia and Ambivalence) assess schizotypal traits in the general population and the possible risk of developing disorders embedded in the psychotic spectrum. Additionally, this tool contains a scale of Hypomaniac Personality to detect risk of affective spectrum psychosis. METHODOLOGY: The Wisconsin Scales of Vulnerability to Psychosis have been adapted following the back-translation method. The scales were translated to an original Spanish version, which was then translated again into English in order to assess the conceptual and semantic overlap with the original items. RESULTS: All the items were back-translated and evaluated in respect to the original ones by an expert in the scales, and all of them were rated with a perfect equivalence (Type A) or satisfactory (Type B). CONCLUSIONS: The Spanish version of the Wisconsin Scales of Vulnerability to Psychosis shows good equivalence with the English one, thus allowing to measure affective and schizotypal traits in samples of Spanish-speaking individuals properly. Future studies should test the reliability and validity of these scales in our environment.

Introducción. Actualmente el estudio de la vulnerabilidad a la psicosis y la intervención temprana es un área de gran impacto clínico e investigador. El objetivo del presente trabajo fue la adaptación de las Escalas de Vulnerabilidad a la Psicosis de Wisconsin del inglés al castellano, un conjunto de cinco escalas (Ideación Mágica, Percepción Aberrante, Anhedonia Física, Anhedonia Social Revisada y Ambivalencia), que valoran rasgos esquizotípicos en población general y, por ende, el posible riesgo de desarrollar patología del espectro psicótico. Adicionalmente, este instrumento comprende una escala de Personalidad Hipomaníaca para detectar riesgo de desarrollo de psicosis del espectro afectivo. Metodología. Las Escalas de Vulnerabilidad a la Psicosis de Wisconsin han sido adaptadas siguiendo la metodología de traducción-retrotraducción. Así, se tradujeron las escalas originales a una versión castellana, la cual volvió a traducirse al inglés con el objetivo de valorar la coincidencia semántica y conceptual con los ítems originales. Resultados. Todos los ítems retrotraducidos y evaluados respecto a los originales por un experto en las escalas recibieron una valoración de equivalencia perfecta (tipo A) o satisfactoria (tipo B).Conclusiones. La versión castellana de las Escalas de Vulnerabilidad a la Psicosis de Wisconsin muestra una buena equivalencia respecto a la escala inglesa, permitiendo así medir los rasgos esquizotípicos y afectivos en muestras hispanoparlantes de forma adecuada. En futuros estudios se deberían comprobar la fiabilidad y validez de estas escalas en nuestro medio (AU)

RESUMO

La pancreatitis autoinmune es una reciente entidad a tener en cuenta en el diagnóstico diferencial de pancreatits crónica-masa pancreática. La presencia de estenosis difusa e irregular del Wirsung, junto con niveles elevados de IgG4, presencia de determinados autoanticuerpos e infiltración linfoplasmocitaria de páncreas son la clave del diagnóstico. La sospecha diagnóstica precoz y la colaboración multidisciplinar es vital en estos casos, ya que puede evitar cirugía innecesaria, puesto que el diagnóstico diferencial se establece con entidades entre las que se encuentra cáncer de páncreas, pancreatitis crónica, cirrosis biliar primaria o colangitis esclerosante primaria. Además, la pancreatitis autoinmune tiene excelente respuesta a tratamiento esteroideo, con resolución completa de parámetros clínicos, analíticos y radiológicos, como ocurría en nuestro paciente. Debido al escaso número de casos descritos en la Literatura, sería necesario la elaboración de estudios con seguimiento largo plazo para conocer pronóstico y frecuencia de asociación a otras patologías

We compared the antimicrobial co-resistance of 3,402 clinical isolates of Enterobacteriaceae resistant to third-generation cephalosporins (2,569 ESBL-producing and 833 AmpC overproducing) with that of 16,220 susceptible isolates, in order to determine the impact of resistance to third-generation cephalosporins on the likelihood of resistance to other antimicrobial classes. Enterobacteriaceae resistant to third-generation cephalosporins, independently of their mechanism of resistance, were significantly more resistant to other classes of antimicrobials than susceptible isolates (p <0.001). Percentages of co-resistance to ciprofloxacin, gentamicin, tobramycin and trimethoprim-sulfamethoxazole of resistant isolates were: 51%, 39%, 53% and 51%, respectively. However, among the susceptible isolates, percentages were 17%, 7%, 6% and 19%, respectively. Fosfomycin exhibited excellent in vitro activity against urinary isolates (92%), mainly against ESBL-producing organisms (90%), and is a good alternative treatment of infections caused by multidrug resistant Enterobacteriaceae. Amikacin and imipenem were the most active antimicrobials against all species tested.

Fundamento. Determinar si las actividades de seguimiento farmacoterapéutico a pacientes crónicos polimedicados,como parte de la atención farmacéutica realizada por farmacéuticos comunitarios, modifica las distintas dimensiones de la calidad de vida percibida. Métodos. 110 pacientes polimedicados fueron aleatorizados ofertándosele a la mitad el seguimiento farmacotera-péutico en la farmacia comunitaria, y a la otra mitad atención habitual. La oferta de seguimiento farmacoterapéutico se hizo en 8 farmacias de la provincia de Sevilla durante 6 meses, cuyos titulares son farmacéuticos específicamente entrenados en actividades de atención farmacéutica. La variable resultado principal es la calidad de vida percibida, medida según el cuestionario SF-36 (versión española). La evaluación se hizo por investigadores independientes.Resultados. A los 6 meses, no se observan diferencias estadísticamente significativas en ninguno de los ochocomponentes de la calidad de vida (función física, rol físico, dolor corporal, salud general, vitalidad, funciónsocial, rol emocional y salud mental). Comparativamente con los valores nacionales de referencia, los pacientescrónicos polimedicados tienden a manifestar peor calidad de vida que la media nacional de referencia,especialmente las mujeres en las dimensiones de rol físico, dolor corporal, vitalidad, función social y saludmental (p<0,05).Conclusiones. La calidad de vida de los pacientes polimedicados es peor que la media nacional. El seguimiento farmacoterapéutico desde las oficinas de farmacia no demuestra mejoría


Objective. To determine whether pharmacotherapeutic follow-up activities in chronic patients on multidrugtherapy, as part of the pharmaceutical care provided by community pharmacists, modifies the dimensionsof perceived quality of life.Methods. The study involved 110 patients on multidrug therapy. Half of them were randomized to undergopharmacotherapeutic follow-up in the community pharmacy and the other half to regular care. The offer ofpharmatherapeutic follow-up involved eight pharmacies in the province of Seville over a six-month period.The proprietors were pharmacists specifically trained in pharmaceutical care activities. The main outcomevariable was the perceived quality of life, measured according to the Spanish version of the SF-36 questionnaire. The evaluation was carried out by independent investigators.Results. After six months of study, no statistically significant differences were observed in any of the eightquality of life components (physical functioning, physical role, bodily pain, general health, vitality, social functioning, emotional role and mental health). When compared with the national reference values, chronicpatients on multidrug therapy tended to present a lower quality of life than the national average. The mostmarked differences were found in women with respect to physical role, bodily pain, vitality, social functioningand mental health (p<0.05).Conclusions. The quality of life in patients on multidrug therapy is lower than the national average. Pharmacotherapeutic follow-up carried out in pharmacies is not associated with an improvement

La endoftalmitis bacteriana endógena es una patología poco frecuente con un pobre pronóstico visual, que requiere un diagnóstico y tratamiento precoz. Presentamos el caso de una paciente diabética que presentó una bacteriemia por Escherichia coli de origen urinario, con endoftalmitis endógena, meningitis y espondilitis secundaria. Fue tratada con antibioterapia intravenosa y tópica, consiguiendo un cese en la progresión y conservando la capacidad para percepción luminosa en el ojo afecto (AU)

We present the case of a patient with an intracerebral hemorrhage as complication of an infectious endocarditis (EI) produced by Neisseria sicca on a prosthetic mitral valve. The patient was anticoagulated previously with Acenocumarol as prophylaxis of thrombosis of the prosthetic valve. He was diagnosed as having IE and later he presented neurological symptoms as consequence of several intracerebral hemorrhagic foci. We decided to continue the anticoagulation with sodium heparin followed of Bemiparina and no new hemorrhagic complications nor thrombosis of the valve were observed after three months of the event. We have not found in the literature any case where low molecular weight heparin has been used as method of anticoagulation in these cases beyond two weeks.

RESUMO

Presentamos el caso de un paciente con una hemorragia intracerebral como complicación de una endocarditis infecciosa (EI) producida por Neisseria sicca sobre una válvula mitral metálica. El paciente estaba previamente anticoagulado con Acenocumarol como profilaxis de trombosis de la válvula protésica. Fue diagnosticado de EI y posteriormente presentó síntomas neurológicos como consecuencia de varios focos hemorrágicos intracerebrales. Se decidió continuar la anticoagulación con heparina sódica inicialmente y posteriormente con Bemiparina, no observándose nuevas complicaciones hemorrágicas ni trombosis de la válvula a los tres meses del evento. No hemos encontrado en la literatura ningún caso donde la heparina de bajo peso molecular (HBPM) haya sido utilizada como método de anticoagulación en estos casos más allá de dos semanas (AU)

The bacterial endogenous endophthalmitis is a slightly frequent pathology with a poor visual prognosis that needs a precocious diagnosis and treatment. We present the case of a diabetic patient who presented a Escherichia coli bacteremia of urinary origin, with endogenous endophthalmitis, meningitis and spondylitis. She was treated with intravenous and topic antibiotics, obtaining a cessation in the progression of the disease and preserving the capacity for luminous perception in the eye.
